ADDENDUM II:
ELECTRONIC STATEMENT(S) AUTHORIZATION
This service will allow you to view your current and previous Account statements for all of your accounts on our secure website at no cost to you.
Authorization
You understand that pursuant to your deposit account and/or loan agreements with Provident Bank, you are entitled to receive periodic account statements and notices in connection with your deposit account and/or loan. By indicating your acceptance of these terms and conditions, you authorize Provident Bank to discontinue sending you such periodic Account Statements and Notices via the U.S. Postal Service for your Accounts and to provide these periodic Account Statements (“Account Statement(s)”) and Notices to you electronically as an E-Statement(s) through ProvidentConnect Online Banking. You understand and agree that going forward Provident Bank will send you an e-mail to your e-mail address on file with us notifying you that your E-Statement(s) and/or Notice(s) are ready to be viewed on the ProvidentConnect Online Banking. You understand and agree that you will no longer receive paper periodic Account Statements and Notices.
You further understand and agree that:
Legally Binding – All Account Owners
You are the account owner of the Account(s) being accessed. Your enrollment executed in conjunction with this authorization given to Provident Bank to deliver all future account statements and notices electronically and any electronic transaction on this Account(s) shall be legally binding on all owners on the account(s) including yourself and that your enrollment and use of E-Statement Services shall be considered authorized by all account owners and/or obligors.
Joint Accounts
If any Account identified by its respective account number within this authorization is in two or more names, it is a joint Account. Each joint owner has the right to enter into this Agreement with us and otherwise give us instructions regarding the Account. If we receive conflicting instructions, we will have the right to refuse to act on either instructions until such time as there is concurrence amongst the joint owners or a final court order or a release acceptable to us and signed by all joint owners. Each joint owner appoints the other as his or her agent to enter into agreements and to conduct business for the Account. The joint owner whose e-mail address we have on our records for the Account agrees to provide to the other joint owner(s) any notice we send to that e-mail address, and notice to one joint owner shall be notice to all.
On-Going Consent and Withdrawal
Your ongoing consent to receive subsequent Account Statements and Notices electronically is voluntary and may be withdrawn at any time. You may withdraw consent at any time by logging in to your account through online banking and deselecting E-statements, in writing, by telephone or by sending an e-mail at (see “How To Contact Us”) and is subject to our confirmation of your request; such authorization withdrawal shall become effective no later than fifteen (15) days after our receipt. Upon termination, you understand and agree that upon our discretion you will no longer have online access to your Account Statements and Notices (previous, current, and future account statements and notices) and that you will receive all Account Statements and Notices in paper form via the U.S. Postal Service, subject to any fee as set forth in the Fee Schedule.
E-Mail Address Updates
You agree to maintain a valid and active e-mail address. You must promptly notify Provident Bank of any change in your e-mail or postal address by contacting Provident Bank via logging into ProvidentConnect Online Banking to update your contact information, writing to us (see “How To Contact Us”) or at any of Provident Bank’s branch locations. Provident Bank is not liable for any third party-incurred fees, other legal liability, or any other issues or liabilities arising from E-Statements or notices sent to an invalid or inactive e-mail address or Account Statements and Notices delivered to a postal address that you have provided.
Paper Statement Requests
At any time, you may also request a paper copy of any periodic Account Statement(s). A fee may be charged for paper copies, subject to the fee as set forth in our Fee Schedule.
Unauthorized Access Notification
You agree to notify Provident Bank immediately if you believe any of your accounts have been accessed or an Access Code has been used without your permission to access your E-Statements.
Cancellation
You understand you may cancel this authorization at any time upon written request at which time Provident Bank will resume mailing of all your future periodic statements and you will no longer have access to your statements online.
Equipment and Software Requirements
In order to use Digital Banking, you will need the type of personal computer, tablet, or telephone, as well as any related equipment and software as determined by Provident Bank from time-to-time. In this Agreement, your personal computer, tablet, telephone, mobile device and/or the related equipment are referred to together as your "Equipment".
You are responsible for the installation, maintenance, and operation of your Equipment and your software. Provident Bank is not responsible for any errors, misuse or failures from any malfunction of your Equipment or your software. Provident Bank is not responsible for any personal computer virus, malware, malicious code, other unauthorized alterations of your software, or equipment take over-related problems that may be associated with the use of Digital Banking. Provident Bank shall have no liability to you for any damage or other loss, direct or consequential, which you may suffer or incur by reason of your use of your Equipment or your software.
